CHaD’s Complex Care Program bridges the gap between specialists, primary care physicians, and families. Whether you are currently in the transitional long-term care program or new to multidisciplinary care, our team provides a flexible, supportive program that allows ease of care and peace of mind for medically complex pediatric patients and their families.
