Manchester Patient Family Advisory Council

Dartmouth Hitchcock Clinics Manchester primary care, in partnership with New Hampshire Family Voices, is excited to offer a Patient Family Advisory Council (PFAC) to members of our community. We recognize quality feedback improves our ability to provide personalized, comprehensive and collaborative care.

The PFAC represents the patient voice and a formal structure for collaborating with patients and their families to improve quality, education, communication, and the patient experience in the health care setting.

Membership

The PFAC in Manchester is currently composed of eight patient and family advisors, two clinicians and one senior leadership member. We are always looking for other motivated individuals, including teens, to be part of the PFAC. Contact us for more information!

Meetings

The PFAC meets once per month on the first Monday of the month, 5:30 to 7:00 pm at Dartmouth Hitchcock Clinics Manchester.
