Discharge planning begins shortly after admission to the hospital.
Your health care team, including the family, will carefully consider the anticipated needs of the your child and will update those plans during the course of his or her hospitalization.
On the day of discharge, the doctor will complete the your child's discharge paperwork which includes:
- The plan for follow up with your child's primary care physician and/or specialty service
- Prescriptions for medications
- Any special care instructions for the your child
Your nurse will review these instructions with the you and provide a written copy.
Our goal is for patients to be discharged in the morning. We will do our best to make that possible for all. Unfortunately, sometimes there are unexpected delays, and for that we apologize. We want you to know that CHaD is actively working on improving the coordination and communication between patients, families, and the health care team on the day of discharge.
If you have questions about the discharge instructions after you go home, please call the telephone number on the discharge instruction sheet.
After you go home, you may receive a telephone call or a survey in the mail, asking you about the care you received while in the hospital. Please help us by answering the questions. Information from these surveys helps us to continually improve care.
